Come work with our skilled interdisciplinary team to provide care and caring services to our patients, families, and staff. Complete nutrition assessments and MDS components, nutrition education, and in-service education to meet the needs of patients, family members, and staff. Complete routine monitoring of patients at high nutrition risk. Collaborate with Dining Services Manager to perform routine quality assurance audits to evaluate food services and nutritional systems and develop plans for improvement. Assist Dining Services Manager in maintaining menu management software system and electronic patient profiles.

The successful candidate will be a Registered Dietitian, qualified through education and experience, with administrative and clinical skills, strong communication skills, and an excellent customer service attitude. Perfer RD with experience in a short term rehab and long term setting, but will train.

The Orange County Health Department is seeking a Registered Dietitian who is looking to advance his/her leadership potential. This unique position provides direct client services (Medical Nutrition Therapy and Diabetes Self-Management Education (DSME)) and coordinates employee wellness and outreach initiatives. Candidates should be highly motivated, possess excellent interpersonal skills and have experience with program planning, implementation and evaluation.

*This is a safety sensitive position and requires a pre-employment drug screen.

EXAMPLES OF DUTIES: A position may not include all the work examples given, nor does the list include all that may be assigned. 

  • Coordinates the daily operations of established DSME program: Recruitment, monitoring client status, data management, delegating appointments and medical records follow-up for educators. 
  • Ensures the DSME program meets the NC Diabetes Education Recognition Program (NC DERP) requirements, attends monthly webinars, communicates updates to staff, and compiles
  • monthly reports. 
  • Leads required quality improvement activities to improve DSME program efficiency and effectiveness. Provides direct client care for all clients enrolled in the DSME program to include: initial assessments, group education component, and follow-up appointments.
  • Provides Medical Nutrition Therapy for clients referred. 
  • Plans and provides employee wellness initiatives for County Employees. 
  • Coordinates and provides client recruitment and outreach opportunities with an emphasis on targeting local medical providers/medical practices. These opportunities include planning and conducting medical provider presentations, providing continued follow-up to maintain relationships with referring medical providers, and ensuring offices have resources needed. 
  • Collaboratively updates program protocols with manager.

Distinguishing Factors
This classification is distinguished from the Registered Dietitian classification by the coordination responsibilities for the DSME program, outreach and employee wellness initiatives.

Knowledge, Skills, Abilities
Thorough knowledge and skill in comprehensive nutrition assessment and care planning, including problem diagnosis, counseling, and evaluation of clients from diverse backgrounds and pages, with a multitude of complex, high-risk medical needs (Examples: diabetes, lipid management, hypertension, high-risk pregnant women and children).

Thorough knowledge and skill in theoretical principles and practices of normal and clinical nutrition and Dietetics, particularly in areas of disease pathophysiology; Advanced, in-depth expertise in a clinical specialty area may be required.

Thorough knowledge in case management and care coordination, health care ethics, documentation in the medical record.

Thorough knowledge and skill in interviewing, counseling and behavior change strategies.

Requires the ability to apply principles of logical thinking to define problems, collect data, establish facts and draw valid conclusions; to deal with several abstract and concrete variables.

Must be able to communicate effectively and efficiently with persons of varying educational and cultural backgrounds.

Thorough knowledge of the organization and delivery of medical care in the community, of medical terminology, and of the roles of the various members of the interdisciplinary health care team.

Skill in monitoring and maintaining quality of nutrition standards and practice.

Strong knowledge of current scientific information regarding nutrition, diet, and health and the ability to relate this information to clients, professionals, and the general public.

does not supervise.
Minimum Qualifications

Registered Dietitian with the Commission on Dietetic Registration with a Master's degree in Dietetics, Public Health Nutrition or Nutrition from an accredited college or university and one year of experience; or a bachelor's degree from an accredited college or university and three years of nutrition experience.

Must be a Licensed Dietitian/Nutritionist with the North Carolina Board of Dietetics/Nutrition. Requires possession of or ability to readily obtain a valid driver's license issued by the State of North Carolina for the type of vehicle or equipment operated.

Physical Activities
Requires work that involves standing, sitting, walking, bending much of the time. Position involves exerting up to 30 pounds of force on a recurring basis, or skill, adeptness and speed in the use of fingers, hands or limbs on repetitive operation of mechanical or electronic office or shop machines or tools within moderate tolerances or limits of accuracy. 

Examples of physical activities required in this position:

  • Sitting at a computer for many hours per day
  • Filing papers
  • Walking to work station
  • Possible Hazards (chemicals, dangerous machinery, etc.)

Health Coach (Full-Time)

Position Summary
The Health Coach engages patients/teammate and encourages them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors. Help patients/teammate unlock personal motivation to make lasting changes and simultaneously serve as the patients/teammates primary conduit to the health care provider as they help coordinate necessary services as needed.

Essential Functions and Responsibilities

  • Performs Annual Wellness Visits (AWV) for eligible patients.
  • Performs chart audits before conducting AWV.
  • Maintains open lines of communication with providers needed for follow up with patients.
  • Communicates with Chronic Care Managers to ensure patient follow up and continuity of care.
  • Schedules follow up appointments as needed.
  • Assists in marketing of wellness services both internally and externally.
  • Assists in development of patient education materials and programs.
  • Identifies resources available to patient population.
  • Assists in successfully reaching quality performance measures.
  • Attends monthly team meetings.
