SALARY: $49,186.00 - $64,890.00 Annually 

GENERAL DESCRIPTION:
The Orange County Health Department has an opening for a Registered Dietitian Program Coordinator.  The work involves the responsibility for providing direct client services (Medical Nutrition Therapy and Diabetes Self-Management Education (DSME)) and coordinating daily operations of the DSME program as well as conducting employee wellness and outreach initiatives. Coordinating the daily operations of the DSME program includes recruitment, monitoring client status, data management, appointments/classes and medical records follow-up for educators.

Work is performed under the general direction of the Nutrition Program Manager with considerable leeway to exercise independent judgment within the framework of applicable rules, regulations, policies and procedures in the performance of duties, both in terms of planning and accomplishing work, and in making decisions regarding customer needs and requests and conducting project assignments. Supervision over others is not required of this position. Position does related work as required.

Examples of Essential Functions 
Following are examples of typical work activities and responsibilities.   The incumbent may be responsible for performing other related duties and responsibilities as required or assigned.  A position may not include all the work examples given, nor does the list include all that may be assigned.
 
Coordinates the daily operations of the DSME program as well as conducting employee wellness and outreach initiatives.
 
Ensures the DSME program meets the NC Diabetes Education Recognition Program requirements, attends monthly webinars, leads required quality improvement activities to improve DSME program efficiency and effectiveness and monitors and updates Quality Coordinator Binder.  
 
Provides direct client care for all clients enrolled in the DSME program to include: initial assessments, group education component, and follow-up appointments.
 
Provides Medical Nutrition Therapy for clients referred. Develops a plan of care for clients, identifying individual problems and risk.  

Plans and provides employee wellness initiatives for County Employees.
 
Coordinates and provides client recruitment and outreach opportunities with an emphasis on targeting local medical providers/medical practices. These opportunities include planning and conducting medical provider presentations, providing continued follow-up to maintain relationships with referring medical providers, and ensuring offices have resources needed. 

Assists in implementing the minority diabetes prevention program.  Duties include: recruitment, protocol development, data collection and reporting, class lesson development, implementation and evaluation. 

Assists in updating program protocols.
 
Assists in grant writing and reporting process. Keeps track of grant deliverables, as well as monthly and quarterly reporting of grant inputs, processes, and outcomes.
 
Ensures billing forms are submitted accurately to reflect MNT and DSME appointments provided.   Ensures all Medicaid and third party reimbursement rules are followed.  
 
Uses computer applications or other automated systems such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.
 
Adheres to the department’s safety rules and regulations.
 
FULL PERFORMANCE KNOWLEDGE, SKILLS AND ABILITIES
Thorough knowledge in comprehensive nutrition assessment and care planning, including problem diagnosis, counseling, and evaluation of clients from diverse backgrounds and ages, who have a multitude of complex, high-risk medical needs (Examples: diabetes, lipid management, hypertension, high-risk prenatals and children).
 
Thorough knowledge in the theoretical principles and practices of normal and clinical nutrition and dietetics, particularly in areas of disease pathophysiology;
 
Thorough knowledge in case management and care coordination, health care ethics, and documentation in the medical record.
 
Thorough knowledge in interviewing, counseling and behavior change strategies and techniques.
 
Thorough knowledge of current research findings and trends in nutrition, dietetics, healthcare, and public health.
 
Thorough knowledge of the organization and delivery of medical care in the community, of medical terminology, and of the roles of the various members of the interdisciplinary health care team.

MINIMUM QUALIFICATIONS 
Graduation from an accredited college or university with a Master’s degree in Dietetics, Public Health Nutrition or Nutrition AND one (1) year of work experience in program planning, implementation and evaluation of nutritional programs; OR
 
Graduation from an accredited college or university with a Bachelor’s degree in Dietetics, Public Health Nutrition or Nutrition AND three (3) years of work experience in program planning, implementation and evaluation of nutritional programs.  
 
Special Requirements
Advanced, in-depth expertise in a clinical specialty area may be required.
 
Must be a Licensed Dietitian/Nutritionist with the North Carolina Board of Dietetics/Nutrition and a Registered Dietitian with the Commission on Dietetic Registration.
 
Possession of or ability to readily obtain a valid driver’s license issued by the State of North Carolina for the type of vehicle or equipment operated.

PHYSICAL REQUIREMENTS AND POSSIBLE HAZARDS
Must be able to physically perform the basic life operational support functions of standing, walking, stooping, keyboarding, talking, sitting, bending, kneeling, grasping, hearing, handling, pushing, pulling, lifting, carrying, reaching and repetitive motions of the hands and wrists.
 
Must be able to physically crawl in confined work areas such as under desks and in wiring closets.
 
Must be able to perform medium work, exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 15 pounds of force constantly to move objects. 

Must possess the close visual acuity to operate a computer terminal and perform extensive reading.

At Atrium Health Anson the overall mission is to improve the health status of Anson County. The Registered Dietitian at Atrium Health Anson plays a pivotal role in promoting and coordinating initiatives. Atrium Health Anson is based upon a medical home model of care which includes an emergency room, primary care office, Med/Surg unit and provides interdisciplinary services all under one roof. The registered dietitian will be responsible for inpatient, outpatient, and community work to address the overall mission of the facility. They will have the opportunity to collaborate with community partners to address food insecurity and educate the community on a number of different health topics. They will play a role in the community needs assessment and the state of the county report as well as opportunities to serve on different community boards and advisory councils.  
 
Diabetes is extremely prevalent in the community and AH Anson is working to change the narrative in which care is provided to those with diabetes. The registered dietitian will partner with the Diabetes Care and Education Specialist to teach diabetes self-management education classes. In addition, work is being done to innovate the way care is delivered with interdisciplinary team members working to develop a new clinic for patients with diabetes. The purpose is to lower HbA1c levels and to promote long lasting disease control through diet education, medication management, and mental health support.  
 
Atrium Health Anson is looking for an independent dietitian who has a passion for community work. The candidate should be willing to work in a group setting but able to thrive independently. The role will include inpatient, outpatient and community work and the registered dietitian should be able to balance these tasks daily. Applicants are expected to be flexible with tasks at hand and be able to pivot as needs arise within the facility and the team.
